1. Application form(006/DGP/DSA);
2. A clear copy of the identification document of the applicant (with both sides of the document printed on the same page of an A4 paper).

Monday to Friday, 9:00 a.m. to 1:00 p.m. and 2:30 p.m. to 5:00 p.m. (closed on Saturdays, Sundays and public holidays)
