How to process
Required Document:
For change of particulars of license holder:
1. Signed declaration for change of information pertaining to the respective license of the license holder;
2. Legible photocopy of respective license;
3a. Legible photocopies of identity document(s) of the original license holder and of the new license holder; or
3b. Legible photocopy of the M7-Business Tax Form issued by the Finance Services Bureau; or
3c. Legible photocopy of the M8-Business Tax Form issued by the Finance Services Bureau;
4. For change of address, the license holder is required to submit supporting documents bearing the address of the place where the animal is kept (for e.g. water bill or electricity bill).
For change of license holder:
1. Declaration for information update for respective license signed by the license holder (or authorized person) or by the applicant;
2. Legible photocopy of respective license;
3a. Legible photocopies of identity document(s) of the original license holder and of the new license holder; or
3b. Legible photocopy of the M7-Business Tax Form issued by the Finance Services Bureau; or
3c. Legible photocopy of the M8-Business Tax Form issued by the Finance Services Bureau;
4. Supporting documents bearing the address of the place where the new license holder keeps the animal(s) (for e.g. water bill or electricity bill).

Monday to Friday, 9:00 a.m. to 1:00 p.m., 2:30 p.m. to 5:00 p.m. (closed on Saturdays, Sundays and public holidays)
