How to Process

Documents required to be submitted:
1.Designated form;
2.Declaration on loss of Registration Card of Veterinary Surgeon.
Document required to be produced:
No document is required to be produced.

4.Islands District Public Services Centre: Rua de Coimbra, n.º 225, 3.˚ andar, Centro de Serviços da RAEM das Ilhas, Taipa

Monday to Friday, 9:00 a.m. to 1:00 p.m., 2:30 p.m. to 5:00 p.m. (closed on Saturday, Sunday and public holidays)

Important notes
The expression “Segunda Via” (second copy) is stated on the reissued registration card and it has the same valid period as the original registration card.

Progress enquiry and collection of result of service

Methods for Collection of Service Result
Collection in person:
The applicant is required to produce the original receipt and the original identification document for verification.
Authorising a representative for collection:
If the applicant intends to authorise another person to collect the service result on behalf of him / her, the representative is required to produce the original receipt and the original identification document for verification. In addition, an authorisation letter is required to be submitted along with a copy of the identification documents of the authoriser and the authorised person.
