How to Apply?
Application Period：Applications are accepted throughout the year.
Application Procedures and Documents Required：
Complete the application form and submit it together with the copy of the applicant’s identification document to the Education and Youth Development Bureau; the form can be obtained at the DSEDJ or downloaded from the DSEDJ website.

The declaration can be obtained within 15 working days from the next day of submitting the completed application form and required documents. (Service commitment)
Enquiry on the Process of the Application and Obtaining the Service Results
Obtain the Service Results：
Collect in person: the applicant needs to produce his/her original identification document.
Collect by an authorised person: the authorised person must submit a copy of his/her identification document, a copy of the applicant’s identification document as well as a written authorisation of the applicant.
