Public authorities (except where a cooperation agreement has been signed).
Time of application
Monday to Thursday: 9:00am to 1:00pm, 2:30pm to 5:45pm
Friday: 9:00am to 1:00pm, 2:30pm to 5:30pm
Closed on Saturdays, Sundays and public holidays
Application procedures and documents required
1. The applicant completes the〔Chemical and Microbiological Testing Request Form〕 according to the “Scope of Testing Service – Bacteriology and Chemistry”.
2. Upon completion of the laboratory test, a paper-based report will be issued to the applicant;
3. If a replacement report is needed, please submit a completed [Application Form for Re-issue of Test Report by the Public Health Laboratory].
Location of Application and Service Hours
In-person application
Apply by completing the corresponding laboratory request form and submitting the collected specimen according to the requirements. A paper-based test report will be available for collection in person at a specific time.

Regulations and guidelines for staff
- All laboratory personnel are required to observe the following laws and regulations:
- Law no. 6/2010 dated 30th August – Rank and Grade System for Pharmacists and Senior Health Technical Officers;
- Law no. 7/2010 dated 30th August – Rank and Grade System for Diagnostic and Therapeutic Technicians.
