Application at counter (applicants are required to obtain a number tag in advance. For the methods to obtain the tag number, please refer to “Methods to Obtain Tag Number and Appointment” of the website of the Identification Services Bureau.)
Target Applicants
Persons who are required to complete Part Four of the “Declaration Form of Assets and Interests”.

Application formalities and required documents
Application can be lodged at DSI in person or by an authorized representative with the following documents:
a. A completed “Application Form for Access to Association Information”;
b. A photocopy of the applicant’s valid identity document.
