Required Document:
1. Declaration for cancellation of respective license signed by the license holder (or authorized person);
2. Legible photocopy of respective license;
3a. Legible photocopy of Export Declaration; or
3b. Death Declaration of the animal(s);
4a. Legible photocopy of identity document(s); or
4b. Legible photocopy of the M7-Business Tax Form issued by the Finance Services Bureau; or
4c. Legible photocopy of the M8-Business Tax Form issued by the Finance Services Bureau.
