How to process
Required Document:
- Declaration – Relationship with the deceased (011/DHA/DHAL);
- A clear photocopy of the identification document of the applicant;
Documents to be presented:
The original identification document of the applicant is required to be produced.

Application fee:
- 6-month storage at most for the deceased above the age of 7 (renewal for once only): MOP2,000.00
- 6-month storage at most for the deceased at or under the age of 7 or stillbirth (renewal for once only): MOP1,000.00
Form fee: Not applicable
Stamp duty: 10% of application fee
Security deposit: Not applicable

Important note:
Statement of reasons for the application for renewal is required to be made by the original applicant. IAM may grant an exception for an extension of storage up to 6 months.
