   1. Original of valid passport/travel document or identification document of minor and all guardians;
   2. Proof of relation between minor and guardians;
   3. Application Form for Travel Restriction of Minor.

Investigation and Repatriation Division of Border Control Department

Office Hours:

Mon to Thu     9:00 – 17:45 (tags dispensed until 17:00)
Fri                     9:00 – 17:30 (same as above)
Closed on Sat, Sun and government holidays
