Application procedures and documents required
For application:
- Original and a copy of the certificate issued by a Health Bureau doctor;
- Copy of the applicant’s Macao SAR Resident Identity Card (present the original);
- One recent photo (1-inch) of the applicant;
Note: Upon staff’s receipt and verification of the relevant documents, the applicant will receive a certified copy of the certificate, whereby the applicant is temporarily entitled to free healthcare during the processing period.
Location and Service Hours for Appointment-Making
- Registration Office, lobby of Conde de São Januário General Hospital, Monday to Friday (except government holidays), 08:30~21:00
- Registration Office, G/F Outpatient Building of Conde de São Januário General Hospital, Monday to Friday (except government holidays), 08:30~13:00, 14:00~17:45 (Friday until 17:30)

Remarks/Notes for Application
To facilitate the renewal process, applicants should apply for the necessary medical certificate in advance:
- If you are consulting a doctor: apply to the doctor directly;
- If you are not consulting a doctor: apply two months before the expiry date of the Healthcare Card at the Medical Report Reception Counter (located on the G/F of Outpatient Building) (Please specify the purpose as “Apply for Healthcare Card for Special Patients”);
- Hematology-oncology patients: apply at the reception desk of the Hematology-oncology department one month before expiry and pick-up two weeks after application.
Application Status Enquiry and Result Collection
For collection:
- Certified copy of the certificate;
- For renewals, the old Healthcare Card should be returned to the office;
- If collected by a third party, the original identity documents of both parties should be presented.
