Documents Required

1.  Original of medical report(s) issued by Conde de São Januario General Hospital;

2.  Proof of income of the applicant and his/her family member(s) (should be signed and stamped by the responsible person of theircompany, with indication of their remuneration and years of service; in case the person does not have regular employment, a certificate signed by the employer(s) indicating the above content, or a personal declaration stating the job content and income will be required);

3.  Applicant’s and family member(s)’ Social Security Fund contribution records (to be handled by Social Welfare Unit);

4.  Persons reporting unemployment should present a dismissal/separation letter, or a personal declaration;

5.  The applicant’s proof of expenses (e.g. water, electricity and telephone bills, receipt of management fees, rent receipt, mortgage payment, etc. of the recent three months) – one original for each item;

6.  A photocopy of the valid ID, student card (if any), or registration card for disability assessment (if any) of the applicant and his/her family member(s);

7.  Two 1.5-inch recent photos of the applicant;

8.  A photocopy of recent drug prescriptions, laboratory sheets, examination sheets and outpatient appointment slip;

9.  The applicant and his/her family members are required to declare all their assets in Macao, the Mainland China and overseas, including: information of real estates, shops, car parks, stocks/shares, funds or other liquidated assets, and provide bank transaction records (fixed deposit or current account) of the recent six months;

10. Policyholders of individual/group medical benefits / medical insurance are required to submit a copy of the details of the benefits / insurance policy.

Remarks/Notes for Application

  1. This information is for reference only; further documents may be required for application review depending on individual situation;
  2. Upon submission of application, the applicant is required to sign a declaration provided by the Social Welfare Unit;
  3. The Social Welfare Unit will conduct random inspection on the approved cases so as to verify the accuracy of the information provided by the applicant;
  4. During the period of entitlement to medical assistance provided by the Hospital, should the financial status of the applicant or his cohabiting family members change, the applicant must take the initiative to report to the Social Welfare Unit and submit a new financial status document for re-evaluation;
  5. Rent receipts or other leasing documents should clearly contain the contact information of the flat owner; furthermore, the applicant is responsible for informing the flat owner of the provision of his/her data;
  6. Wherever necessary, it is required to present a certificate of property tax payment issued by the Financial Services Bureau, and/or a certificate of commercial registration issued by the Commerce and Movable Property Registry;
  7. Wherever necessary, a declaration of financial status of non-cohabiting family member(s) will be required;
  8. Family members of the applicant, including his/her parents, spouse, children and siblings, are required to undergo the aforesaid financial assessment;
  9.  If necessary, the persons concerned will be contacted for data verification;
  10. The Health Bureau reserves the right to retain all submitted information for record keeping purposes.
