How to Apply
Time of application: Application must be submitted to the Health Bureau before the deadline of premise establishment.
Application procedures and documents required:
- The completed DR-11 Application Form for Extension of ‘Medicine Store’ Establishment Deadline;
- Document(s) justifying the deadline extension (If applicable).

Application Processing Time
The Health Bureau will finish the review and approval process within 90 days upon receipt of all the application documents required.
Remarks/Notes for Application
According to Paragraph 4 of Article 19 of Decree Law no. 58/90/M dated 19th September, the applicant/legal person has six months to establish a premise, and may apply for an extension of the establishment deadline if they can prove that the delay in establishment is not caused by them on purpose.
