How to Apply
Time of application: An application to the Health Bureau is required in order to change the particulars in the licence and registration form of a firm for import, export and wholesale of pharmaceutical products, including the firm name, telephone number and/or fax number as well as the firm owner’s identification document.
Application procedures and documents required:
- Application documents required for change of firm name:
- Completed FI-10 Application Form for Change of Particulars in the Licence and Registration Form of ‘Firm for Import, Export and Wholesale of Pharmaceutical Products’;
- Design of the firm’s signboard (Note 1);
- Photocopy of updated Business Tax Declaration Form (M/1 format) obtained from the Financial Services Bureau (Note 2).
- Application documents required for change of the firm’s telephone number and/or fax number:
- Completed FI-10 Application Form for Change of Particulars in the Licence and Registration Form of ‘Firm for Import, Export and Wholesale of Pharmaceutical Products’;
- Application documents required for change of identification document of the firm owner (Note 3-4):
- Completed FI-10 Application Form for Change of Particulars in the Licence and Registration Form of ‘Firm for Import, Export and Wholesale of Pharmaceutical Products’;
- Certified true copy of updated identification document, or present the original of the aforesaid document (for verification and certification by the Pharmaceutical Affairs Department of the Health Bureau).
Notes:
- The applicant shall alter the firm’s signboard according to the approved signboard design after the application for change of firm name is approved, and shall submit relevant photo(s) to the Pharmaceutical Affairs Department of the Health Bureau;
- Can be submitted after the application for change of firm name is approved;
- Refer to the identification document of an executive management member if the firm owner is a legal person;
- If the owner also holds the licence(s) of other business(es) involved in pharmaceutical activities, please provide a list of the business(es) concerned.

Application Processing Time
The Health Bureau will finish the review and approval process within 90 days upon receipt of all the application documents required.
