Application procedures and documents required
- Documents to be produced
- The original identity document of the applicant;
- The original or certified true copy of the Death Certificate issued by a doctor or an authority from the place of death;

- Documents to be submitted
- A duly completed Request Form of Certificate of Medical Sanitation;
- A photocopy of the death certificate of the deceased;
- A photocopy of the applicant’s identity document
[Not required for applications made in the name of a company / organization that have submitted the document, as long as the identity document remains valid]; - A photocopy of the deceased’s identity document;
- A declaration or proof of appropriate sanitary handling of the body
For import of the body, a copy should be submitted with the application in advance, and the original should be prepared for verification upon the importation of the body
For export of the body, the document should be submitted whenever is required in the Certificate of Medical Sanitation, otherwise it can be submitted prior to sealing the coffin
