Making an Application


How to Apply?

Application Period:Applications are accepted throughout the year.

Application Procedures and Documents Required:
Collect an application form at the Education and Youth Development Bureau or download it from the DSEDJ website. Fill in the application form and submit it together with the copy of the applicant’s identification document.

Processing Time

The certificate can be obtained within 8 working days of receipt of the completed application form and required document(s). (Service commitment)


Enquiry on the Process of the Application and Obtaining the Service Results

Obtain the Service Results:

Collect in person: the applicant needs to produce his/her original identification document.

Collect by an authorised person: the authorised person must submit a copy of his/her identification document, a copy of the applicant’s identification document as well as a written authorisation of the applicant.
