Procedures and required documents
1. Mod. No. 3A (available at the service counters of the Transport Bureau or download from the Transport Bureau website).
2. Original of a valid Macao Special Administrative Region Resident Identity Card or a valid document for legal stay in Macao SAR (in case of evaluation and approval, the document is digitalized and saved for the records)
3. One 1,5 inches recent color photographs (photograph specification) or choose the photo for identification documents under “My photos” in “Macao One Account” ( list of contracted merchants on Macao One Account) of 12 months or less from the shooting date.
4. Original of the Macao Driving License
5. Copy of the certificate of personal data (if applicable, in case of evaluation and approval, the document is digitalized and saved for the records).

Remarks/Application Notes
1. In accordance with the “Macao Traffic Regulation”, supplementary document must be submitted within 90 days upon the date of submission of the application, otherwise, the application has to be reapplied all over again and related payment must be paid.
2. The authorized person is required to show his/her original valid identity card（in case of evaluation and approval, the document is digitalized and saved for the records）.
