How to apply

Deadline for formalities:

The application should be lodged within 3 working days after the birth of the newborn or before discharge. Any late applications will have to process at the Civil Affairs Registry.

Formalities and documents required:

  1. Registration can be made by either parent of the newborn
  2. Originals and photocopies of identity documents of both parents. (Please present the identity document of the puerperant which was registered during admission)
  3. Duly completed Notes for “Comunicação de Nascimento (Birth Notice)” Registration which contains the information required by the Statistics and Census Service

Location of Application and Service Hours

Location: Reception desk of Obstetrics Department, 2/F of the Gynaecological and Paediatric Building of Conde de São Januário General Hospital

Service hours: Mondays to Fridays (except public holidays): 13:00~16:00

Remarks/Notes for Application

For failure to present original of identity document of either parent, or special declaration of registering the birth without the father’s details, the baby’s mother is required to complete a declaration

Collection of Result

The “Comunicação de nascimento (Birth Notice)” for newborn can be processed at the Civil Affairs Registry 5 working days upon completion of the registration process for the Birth Notice in Conde de São Januário General Hospital.
