How to Apply
Submission Period:
Applications are accepted throughout the year.
Application Procedures and Documents Required:
1) Submit the application form to the Graduate School:
Complete the application form for transcript (GRS / Form / 040) and present campus card or identity document. If a representative is authorized to complete the procedures on behalf of the applicant, an authorization letter and the ID copies of both the applicant and the representative are required.
2) Submit the application form by email:
Complete the application form for transcript (GRS / Form / 040) and email it to the Graduate School.

3) Apply via the Online Payment Service system (For registered students and graduates who graduated on/after 6 June 2018)

Status Enquiry and Service Results Collection
Enquiry on application status:
1) Call the Graduate School
2) Email the Graduate School
3) Access the Online Payment Service system (For applications submitted via the Online Payment Service system)
Collection methods of service results:
The applicant can choose one of the following collection methods when submitting the application:
1) Collect/Authorize a representative to collect the document at the Graduate School
2) Print out the document at the designated Self-Service Kiosk
3) Deliver the document to the specified organization by post
Documents need to be presented or submitted upon receipt of the service results:
1) The document can be collected at the Graduate School by presenting the receipt and the applicant’s campus card or identification document. If the document is collected by a representative, then an authorization letter and copies of the identification documents of both the applicant and the representative should be presented.
2) Print out the document at the designated Self-Service Kiosk by scanning campus card or QR code.
