Service Target
- Individuals with a full licence for working as a medical doctor, dentist, traditional Chinese medical doctor, nurse, medical laboratory technologist, radiological technologist, chiropractor, physiotherapist, occupational therapist, speech therapist, psychotherapist, or dietitian, must apply for suspension of their licence if they plan to stop practicing for over six months and apply for cancellation of their licence if they wish to cancel the practice.
- In addition, if the licence holder is under the circumstances stipulated in Subparagraphs (1) to (4) of Article 19 – Registration Cancellation of Law no. 18/2020, they must apply for cancellation of their full licence. The applicable conditions include:
- The health professional has raised such an application;
- The health professional is dead or declared quasi-interdicted or interdicted;
- The health professional registered with false declarations or information or used other unlawful means;
- The health professional is deemed as unqualified for pursuing the profession due to falling into the conditions defined in Subparagraphs (1) to (4) of Paragraph 3 in Article 13.
Application Deadline
- The licence holder must submit an application for suspension or cancellation of their full licence within 30 days following the change to their practicing status.
- Licence holders who need to cancel their registration according to Subparagraphs (1) to (4) of Article 19 of Law no. 18/2020 must submit an application for full licence cancellation within 30 days after obtaining relevant proof issued by Health Professional Council for cancelling the provisional accreditation registration.

- Original Full Licence (If the licence is lost, a copy of statement on loss of the licence issued by the competent authority must also be submitted)
Application for full licence cancellation
- Application form for Suspension or Cancellation of Full Licence for Health Professionals;
- Original Full Licence;
- The following documents must also be submitted under the stated circumstances:
- A copy of statement of the loss of the licence issued by the competent authority if the licence is lost;
- A copy of a death certificate if the licence holder is dead;
- Declaration of Management and Retention of Medical Records by Legal Person Healthcare Service Providers (if any);
- Relevant proof issued by Health Professional Council for cancelling the confirmed accreditation registration, or a certified copy of the proof (if applicable).
Notes:
- The interval of suspension should not exceed two years each time;
- The applicant must remove the signboard of the medical establishment (if applicable)
- If the applicant intending to cancel their licence practices at a medical institution and the institution is responsible for manging and retaining medical records, the applicant can, upon applying for licence cancellation, submit the Declaration of Management and Retention of Medical Records by Legal Person Healthcare Service Providers. If the applicant fails to do so, they must, within 60 days from the following day of receiving the notification letter on licence cancellation, directly give the medical records to the patients or hand them over to the Medical Activity Licensing Division, or submit the Declaration of Management and Retention of Medical Records by Legal Person Healthcare Service Providers.
