Deadline for application: Not applicable

Processing procedure and required documents:

1. Application form (032/DIA/DSA);
2. Clear photocopy of the valid Macao SAR resident identification document of the applicant (with both sides of the document photocopied on the same page).

• Integrated Services Centre: Avenida da Praia Grande, n.^os 762-804, Edifício China Plaza, 2.º andar, Macau;

• Northern District Public Services Centre: Rua Nova da Areia Preta, n.º 52, Centro de Serviços da RAEM, Macau;

• Central District Public Services Centre: Rotunda de Carlos da Maia, n.^os 5 e 7, Complexo da Rotunda de Carlos da Maia, 3.˚ andar, Macau;

• Islands District Public Services Centre: Rua de Coimbra, n.º 225, Centro de Serviços da RAEM das Ilhas, 3.˚ andar, Taipa;

• Islands District Public Services Centre – Seac Pai Van Station: Avenida de Vale das Borboletas, Complexo Comunitário de Seac Pai Van, 6.˚ andar, Coloane.

Office hours:

Monday to Friday, 9:00 a.m. to 6:00 p.m. (no lunch break, closed on Saturdays, Sundays and public holidays)

1. Each application form is limited to application for one item only;
2. Online applications can be processed through “Food Safety Information” website or “Business and Associations Platform”. (Applicants who process the application formalities online must have a “Macao One Account”. Relevant standards or requirements

If the application for cancellation is not submitted by the holder of the registration certificate or his/her representative, the applicant has to submit documentary proof of his/her right to apply for cancellation of registration.
