How to process

Documents required to be submitted:
1.Application form for License for Establishments of Veterinary Clinical Care Activities and Establishments of Commercial Activity of Animals(031/DICV/DIS);
2.If the applicant is an individual, the following documents are required to be submitted:
     2.1Copy of identification document;
     2.2Certificate of Criminal Record.
3.If the applicant is a legal person, the following documents are required to be submitted:
     3.1 For a company
        3.1.1 Copy of business registration certificate issued by the Commercial and Movable Property Registry of Macao;
        3.1.2 Copy of the identification documents of the managers and members of administration and management organs of the company;
        3.1.3 The Certificates of Criminal Record of the managers and members of administration and management organs of the company; 
     3.2 For an association or foundation
        3.2.1 Copy of the certificate of registration in the Identification Services Bureau issued by the bureau;
        3.2.2 Copy of the Certificate of Leadership Structure issued by the Identification Services Bureau;
        3.2.3 Copy of the identification documents of main office-holders of the association or foundation;
        3.2.4 The Certificates of Criminal Record of main office-holders of the association or foundation;
4.Construction plan of the establishment intended for engagement in business;
5.Information and description of the compartments, facilities and equipment in the establishment intended for engagement in business;
6.The license for use of the establishment intended for engagement in business issued by the Land and Urban Construction Bureau or the written report of property registration issued by the Property Registry;
7.Certification document of no any debts subject to coercive collection through tax enforcement procedures issued by the Financial Services Bureau;
8.Copy of the identification document of the veterinary surgeon to be employed;
9.Copy of the registration card of the veterinary surgeon to be employed;
10.Declaration on taking on the position made by the veterinary surgeon to be employed;
11.Management plan of the establishment intended for engagement in business;
12.Other certification documents deemed necessary by the Municipal Affairs Bureau.

The above-mentioned documents are required to be submitted in seven copies. If the establishment intends to employ over 30 employees, an extra copy of the documents is required to be added accordingly. During the process of license application, the Division of Animal Inspection and Control may request the provision of other required information depending on the actual situation.

Document required to be produced:
Original or notarised copy of the identification document of the applicant / legitimate representative with form of signature.

4. Islands District Public Services Centre: Rua de Coimbra, n.º 225, 3.˚ andar, Centro de Serviços da RAEM das Ilhas, Taipa
5. Islands District Public Services Centre – Seac Pai Van Station: Avenida de Vale das Borboletas, Complexo Comunitário de Seac Pai Van, 6.˚ andar, Coloane 

Office hours:
1.  Integrated Services Centre and Public Services Centres
Monday to Friday 9:00 a.m. to 6:00 p.m. (no lunch break, closed on Saturday, Sunday and public holidays)
2.  Islands District Public Services Centre – Seac Pai Van Station
Monday to Friday, 9:00 a.m. to 1:00 p.m., 2:30 p.m. to 5:00 p.m. (closed on Saturday, Sunday and public holidays)

Fees

Application fee: MOP 2,000

Stamp duty: 10% of application fee (MOP 200)


Time required for approval 

If the applicant submits all required documents that meet the standards within the designated period, the time for approval review is 81 working days excluding the construction period.

Remarks / Important notes on application

1. Applicants can make an application in person or by authorising a representative;
2. Whether the procedures can be completed successfully depends on whether the applicant can rectify the deficiencies or provide supplementary information on time.

Progress enquiry and collection of result of service

Method for collection of service result
Collection in person:
The person that collects the result should bring the receipt and produce the original identification document.
Authorising a representative for collection:
If the applicant intends to authorise another person to collect the service result on behalf of him / her, the representative is required to produce the original receipt and the original identification document for verification. In addition, an authorisation letter is required to be submitted along with a copy of the identification documents of the authoriser and the authorised person.
