How to process
Documents required to be submitted:
1. Application form for License for Establishments of Veterinary Clinical Care Activities and Establishments of Commercial Activity of Animals(031/DICV/DIS);
2. If the applicant is an individual, the following documents are required to be submitted:
2.1 Copy of identification document;
2.2 Certificate of Criminal Record.
3. If the applicant is a legal person, the following documents are required to be submitted:
3.1 For a company
3.1.1 Copy of valid business registration certificate issued by the Commercial and Movable Property Registry of Macao;
3.1.2 Copy of the identification documents of the managers and members of administration and management organs of the company;
3.1.3 The Certificates of Criminal Record of the managers and members of administration and management organs of the company;
3.2 For an association or foundation
3.2.1 The certificate of registration in the Identification Services Bureau issued by the bureau;
3.2.2 The Certificate of Leadership Structure issued by the Identification Services Bureau;
3.2.3 Copy of the identification documents of main office-holders of the organs;
3.2.4 The Certificates of Criminal Record of main office-holders of the organs;
4. Copy of New Registration Form of the Financial Services Bureau;
5. Certification document of no any debts subject to coercive collection through tax enforcement procedures issued by the Financial Services Bureau;
6. Copy of valid registration cards of the veterinary surgeons of the establishment;
7. Management plan of the establishment;
8. Other certification documents deemed necessary by the Municipal Affairs Bureau.

Original or notarised copy of the identification document of the applicant / legitimate representative with form of signature

Monday to Friday 9:00 a.m. to 6:00 p.m. (no lunch break, closed on Saturday, Sunday and public holidays)
2. Islands District Public Services Centre – Seac Pai Van Station
Monday to Friday, 9:00 a.m. to 1:00 p.m., 2:30 p.m. to 5:00 p.m. (closed on Saturday, Sunday and public holidays)
Fees
Application fee: MOP 2,000
Stamp duty: 10% of application fee (MOP 200)
Time required for approval
If the applicant submits all required documents that meet the standards within the designated period, the time for approval review is 20 working days.

Applicants can make an application in person or by authorising a representative.

Method for collection of service result
Collection in person:
The person that collects the result should bring the receipt and produce the original identification document.
Authorising a representative for collection:
If the applicant intends to authorise another person to collect the service result on behalf of him / her, the representative is required to produce the original receipt and the original identification document for verification. In addition, an authorisation letter is required to be submitted along with a copy of the identification documents of the authoriser and the authorised person.
