How to process
Documents required to be submitted:
1. Designated form;
2. If the transferee is an individual, the following documents are required to be submitted:
2.1 Copy of identification document;
2.2 Certificate of Criminal Record;
2.3 Document issued by the Financial Services Bureau to certify that the prospective transferee has no debts subject to coercive collection through tax enforcement procedures;
2.4 The transfer contract / assignment / declaration (copy) of the establishment of veterinary clinical care activities concerned.
3. If the prospective transferee is a legal person, the following documents are required to be submitted:
3.1 In the case that the prospective transferee is a company
3.1.1 Copy of valid business registration certificate;
3.1.2 Copy of the identification documents of the managers and members of administration and management organs of the company;
3.1.3 The Certificates of Criminal Record of the managers and members of administration and management organs of the company;
3.1.4 Document issued by the Financial Services Bureau to certify that the prospective transferee has no debts subject to coercive collection through tax enforcement procedures;
3.1.5 The transfer contract / assignment / declaration (copy) of the establishment of veterinary clinical care activities concerned.
3.2 In the case that the prospective transferee is an association or foundation
3.2.1 Certificate of registration in the Identification Services Bureau issued by the bureau;
3.2.2 Certificate of Leadership Structure issued by the Identification Services Bureau;
3.2.3 Copy of identification documents of main office-holders of the organs;
3.2.4 The Certificates of Criminal Record of main office-holders of the organs;
3.2.5 Document issued by the Financial Services Bureau to certify that the prospective transferee has no debts subject to coercive collection through tax enforcement procedures;
3.2.6 The transfer contract / assignment / declaration (copy) of the establishment of veterinary clinical care activities concerned.
4. Other certification documents deemed necessary by the Municipal Affairs Bureau.
Documents required to be produced:
1. Original or notarised copy of the identification document of the applicant / legitimate representative with form of signature;
2. Original or notarised copy of the transfer contract / assignment / declaration of the establishment of veterinary clinical care activities concerned with the content including (but not limited to) the following information:
2.1 The identification information, contact information and others of both parties of the transfer of ownership;
2.2 Information about the establishment, such as license number, establishment name, type and category, and address of establishment;
2.3 Terms of the transfer of ownership;
2.4 Effective date of contract;
2.5 The signatures of both parties of the transfer of ownership are required to be notarised.

Fees
Application fee: MOP 300
Time required for approval
If the applicant submits all required documents that meet the standards within the designated period, the time for approval review is 10 working days.

Method for collection of service result
Collection in person:
The person that collects the result should bring the receipt and produce the original identification document.
Authorising a representative for collection:
If the applicant intends to authorise another person to collect the service result on behalf of him / her, the representative is required to produce the original receipt and the original identification document for verification. In addition, an authorisation letter is required to be submitted along with a copy of the identification documents of the authoriser and the authorised person.
