How to process
Deadline for application: A notification should be made to the Municipal Affairs Bureau within 30 days counting from the day of occurrence of the change of the managers, the members of administration and management organs or the main office-holders of organs.
Documents required to be submitted:
1. Designated form;
2. In addition to the above-mentioned designated form, the following documents are required to be submitted:
2.1 If the applicant is a company:
2.1.1 Copy of valid business registration certificate;
2.1.2 Copy of the identification document of the new manager or member of administration and management organs of the company;
2.1.3 The Certificate of Criminal Record of the new manager or member of administration and management organs of the company.
2.2 If the applicant is an association or foundation:
2.2.1 Certificate of registration in the Identification Services Bureau issued by the bureau;
2.2.2 Certificate of Leadership Structure issued by the Identification Services Bureau;
2.2.3 Copy of identification document of the new main office-holder of organs;
2.2.4 The Certificate of Criminal Record of the new main office-holder of organs.

Monday to Friday 9:00 a.m. to 6:00 p.m. (no lunch break, closed on Saturday, Sunday and public holidays)
2. Islands District Public Services Centre – Seac Pai Van Station
Monday to Friday, 9:00 a.m. to 1:00 p.m., 2:30 p.m. to 5:00 p.m. (closed on Saturday, Sunday and public holidays)
Fees
Payment of fees is not required.
Time required for approval
The time for approval review is 10 working days.
